Inova Health System offers downloadable versions of Your Right to Decide in eight languages. This helpful booklet discusses advance directives and answers frequently asked questions. We also provide downloadable versions of Virginia Advance Medical Directive in eight languages. This form, which you complete and sign, allows you to specify your healthcare choices. Both the booklet and the form reflect current State of Virginia legal code and the requirements of The Joint Commission.
