Applicants must be eligible for Virginia licensure and submit the following application packet by by January 3, 2012.
- Formal letter of interest describing personal and professisonal goals as they relate to clinical pharmacy practice
- Curriculum vitae
- College transcript from an accredited school of pharmacy in a sealed envelope from the Dean's office (can be mailed directly if necessary)
- Three letters of recommendation in sealed envelopes (suggested references include professors, preceptors and/or employers), including this form
- Completed residency application
Qualified applicants will be invited to complete an onsite interview in late January or throughout February. Candidates for our residency program are selected in conjunction with the ASHP Resident Matching Program (the "Match"). Our NMS code is 124713.
