Three Steps to Volunteer

  1. Complete the adult volunteer application (.doc). You may either print out the completed form (to fax or mail to the address below), or you may save it as a Word document and attach and send by email.

  2. Print out two personal reference request forms. Ask two people who are not family members or members of your household to serve as your references and to complete the forms.

  3. Fax or mail your application and reference forms to:
