How do I make an appointment? How soon are appointments available?

To schedule an appointment, call 703-776-7005. Appointments typically are available within 2-4 weeks.

Medical history

An important part of your executive physical is obtaining your comprehensive medical history. We want this process to be convenient and easy. Rather than spending valuable time during your appointment to complete paperwork, please take the time to complete the forms below before you come in for your appointment.

Here is how to complete your medical history

  • Complete online the required forms listed below that are appropriate for you. The forms are all fillable PDFs.  
  • Print out the completed forms
  • Bring all forms with you on the day of your appointment

Please note: Because these forms contain protected medical information as defined by HIPAA, you will be unable to save partially-completed forms and return to them later. Make sure you allow yourself enough time to complete a form and then print it before closing out of the form.

Where does the examination take place?

All medical services and testing are performed at our offices on the campus of Inova Fairfax Hospital, conveniently located just off the Gallows Road exit of I-495.

What physicians will I see as part of Inova Executive Health?

Our team is led by Craig Cheifetz, MD, medical director. John Pace, MD, and Laura Balda, MD,  are lead physicians. As part of the program, you will also interact with a board-certified cardiologist and other physician specialties recommended for your examination.  Learn more about our team arrow

How much time will a health screening require?

Each appointment is based on the recommendations of the physician performing the screening. You should plan on spending about 4–5 hours at the appointment. Optional screenings may require an additional visit.

Is there paperwork I need to complete ahead of time?

Yes, you will need to complete registration and medical history forms. For your convenience, you can use the fillable PDFs online to complete all the necessary patient forms in advance of your appointment.

When will I receive the results from my exam and tests?

You can expect 90% of your results to be available on the same day of your appointment. However, it is highly recommended that you schedule a follow-up, one-on-one consultation (or by phone) 30 days after your visit. We will discuss with you in detail your overall exam results, action plans and goals.

Other frequently asked questions

Will my employer have access to my medical records?

No. All of your medical information is kept strictly confidential and will never be shared with your employer.

Can I share the results of my screening/exam with my primary care physician?

Yes. You will receive a comprehensive summary of the results of your health screening and exam. You are encouraged by Inova to share this information with your physician. Inova can also send your primary physician a copy of your records upon request.

How frequently can I receive a health screening/exam under this benefit?

Exams are provided once a year.

Is there a cost for me to participate in the Inova Executive Health screening program?

If your employer pays for the cost of your health screening, we will send an invoice directly to them. You will not receive any bills from Inova for services administered. If your employer does not pay for the exam, you can pay by credit card at the time of service.
