Technical assistance is available for parish nurses, health ministries and clergy who have signed faith partner agreements with Congregationall Health Partnership. A designated person from the faith community may collaborate with CHP regarding community linkages to meet their health ministry needs.
Congregational needs assessment surveys
Congregational needs assessment surveys are available to partners to explore self-reported health concerns for their congregation. This will help shape education and services provided through their health ministry.
Program evaluation
Congregational Health Partnership can provide technical assistance to partners that wish to evaluate their faith community nurse and health ministry services.
