Overview

Many people face the challenge of caring for a disabled spouse or older family member, often without the information or support they need to cope with the sometimes overwhelming burdens of medical care, social service bureaucracies and financial obligations.

Until recently, they faced this task alone. Now, with ElderLink, there is help and hope.


A unique approach to care

ElderLink was created out of an unusual public-private partnership among Inova Health System, the Fairfax Area Agency on Aging, and the Alzheimer’s Association of Northern Virginia. An exceptional array of resources, programs and services, combined with a well-trained staff of skilled geriatrics specialists, makes us uniquely qualified to address the specific needs of caregivers and clients alike.

Most importantly, ElderLink coordinates and manages all elements of patient care (medical, social, legal and financial), minimizing the stress and confusion families often experience when dealing with multiple service providers.

Some families require minimal assistance. Others require intensive intervention. Whatever the situation, ElderLink will make the most appropriate recommendation based on a careful assessment of patient and family needs.


Fees

ElderLink is a private, not-for-profit organization. In Fairfax County, a flat rate is charged for an assessment, care plan and service coordination. Outside Fairfax County, an affordable hourly rate is charged for care management services. Financial assistance may be available on a sliding fee basis. ElderLink is a not-for-profit program that relies on community support. Tax-deductible contributions from individuals, service groups, and local businesses help us participate in the partnership for the sake of our older neighbors. Donations may be mailed directly to ElderLink.
