New patients, download the necessary registration forms and bring them with you to your first appointment
If you are unable to download the forms, please arrive 20 minutes early to complete the registration forms. Please remember to bring your insurance card and photo ID to each visit. In addition, please bring a list of your medications (prescriptions and over-the-counter) to each visit.
We require 24-hours notice of cancellation or change in your appointment date or time. A cancellation fee will be billed to you for missed appointments, or for appointments not cancelled 24 hours in advance. Please call us if you are going to be late. You may be asked to reschedule.
We accept all major commercial insurance plans and Medicare. Co-pays and co-insurances are due at time of service. We accept cash, checks and all major credit cards.
If you are having a pharmacy call our office for your refill authorization, please contact your pharmacy before 3 p.m., Monday through Friday. This will help ensure a quicker response on our end. There may be a 48-hour turn around on your prescription refill.
Some insurance plans require a referral from your family physician before making an appointment with a specialist. Referral authorizations may take 72 hours to process, so contact us as soon as the need for a referral arises.
