Recognizing the importance of advance directives
None of us likes to think about how we would want healthcare delivered to us if we became critically ill or unable to communicate. But it is important to make these essential and very personal decisions – known as advance directives – before needing them.
Inova offers downloadable versions of Your Right to Decide in eight languages. This helpful booklet discusses advance directives and answers frequently asked questions. We also provide downloadable versions of Virginia Advance Medical Directive in eight languages. This form, which you complete and sign, allows you to specify your healthcare choices. Both the booklet and the form reflect current State of Virginia legal code and the requirements of The Joint Commission.
