None of us really likes to think about the way we would want healthcare delivered to us if we became critically ill or unable to communicate. But it is important to make these important and very personal decisions – known as advance directives – before ever needing them.
Inova Health System offers downloadable versions of Your Right to Decide in eight languages. This helpful booklet discusses advance directives and answers the most frequently asked questions. We also provide downloadable versions of Virginia Advance Medical Directive in eight languages. This form, which you complete and sign, allows you to specify your healthcare choices. Both the booklet and the form reflect current State of Virginia legal code and the requirements of the Joint Commission.
We suggest you share these documents with your family members and provide signed copies to each of your physicians for placement in your medical record.
