Online Medical History
To prepare for your surgery or procedure, we ask that you complete your medical history online. You will use our totally secure One Medical Passport tool. We recommend you do this as soon as your surgery is scheduled. Once completed, our pre-surgical nurse will review your medical history with you during your scheduled interview.
Important details to know
- Completing your online medical history is easy and usually takes less than 30 minutes
- All your information remains completely confidential and is only accessible by our medical team
- You may at any time stop filling out the questionnaire and return to it at a more convenient time
- You may print out a copy of your medical history for your records after creating it
- You may access your online information at any time to review or update the information

Information security
One Medical Passport uses the highest grade of Internet security available: 128 bit encryption with SSL (secure socket layer) technology. Your information is stored on servers at a secure webhosting site and will only be released to your medical team at the Inova hospital where you will be a patient. Your information is never released to anyone other than your medical team. One Medical Passport complies with federal guidelines for protecting patients' personal medical information set forth in the Health Insurance Portability and Accountability Act of 1996 (HEP).
