Service Locations

Inova Reference Laboratory Service Centers are staffed with excellent professional phlebotomists for adult and pediatric specimen collection.

When visiting a Patient Service Center, please bring the following information:

• A lab testing request from a health care professional requesting laboratory testing (prescription or IRL request form)
• A current insurance identification card (Medicare, private insurance or HMO/PPO)
• A photo ID (e.g., driver's license, employee identification badge)
