Directions, Parking and Access
Parking and access. Enter the hospital campus at the Gray entrance off of Gallows Road and park in the Gray parking garage, preferably on ground level. Exit the garage at ground level directly into IHVI. Our offices are located on the right in the Outpatient Clinical Care Center once you enter IHVI.
Valet Parking. Valet parking is available
at the entrance to the Outpatient Surgery
Center. Enter the campus at the Gray Entrance.
Pass the main hospital and turn right at the
entrance to the Outpatient Surgery Center and IHVI.
After leaving your car with the attendant, enter the surgery center, turn left and continue through the glass doors connecting the main hospital to IHVI. Pass the gift shop, coffee kiosk, information desk and water fountain on your left. Our office is through the second door on the left. If you reach the garage you just missed us!
Drop Off. If you are being dropped off, follow directions for valet parking. You may request a wheelchair if needed at the information desk in the lobby.
Travel time. Keep in mind that traffic in Northern Virginia is often very heavy, especially during commute times, and plan accordingly when driving in for your appointment.
Oxygen. Let us know if you need supplemental oxygen during your visit and we will provide it for you.
Insurance. If your insurance company requires a referral for your appointment, please contact your primary care physician to request one. The referral should be made for "Inova Transplant Center Physicians."
Co-payments are due at the time of your visit. If we have not received your referral at the time of your appointment we may ask you to sign a waiver. Should your insurance not reimburse the center you may be responsible for payment. You have an option to reschedule if your insurance does not allow you to sign a waiver.
Forms. Please complete the following forms you received in your information package and bring them to your appointment:
- Pulmonary health questionnaire
- General health history
- Authorization form that designates whom we notify regarding your medical and appointment information
- Authorization form to release/disclose protected health information (highlighted sections only)
Medical Records. It is important for us to receive your prior medical record(s) before your visit. This allows us to organize a chart so your visit is as efficient and thorough as possible. You may need to request copies of slides, films or CD from your local hospital or radiology center, so allow ample time for your request to be filled. We also send letters to your physician(s) requesting your records, but we ask that you contact them as well and ask that your records be sent to us. Following is a list of requested records:
- Recent history and physical
- Consultation notes
- Most recent office visit notes
- Any testing results your referring physician, other specialist physician(s) and/or primary care physician feel may be helpful in determining the best treatment options for you
If performed, the following reports are especially important:
- Actual biopsy slides and pathology report
- Pulmonary function tests
- 6 minute walk test results
- Actual chest X-ray film or CD with report
- Actual chest CT scan film or CD with report
- Cardiac catheterization record
- Echocardiogram report
- Lab work
Registration. When you check in we will verify your registration information. We will make copies of your insurance card(s), photo ID, and ask you to sign a form that allows us to submit a claim to your insurance company.
Vital signs and walking test. One of our clinical staff members will record your weight, blood pressure, heart rate, breathing rate, oxygen level and temperature. Next you will take a 6 minute walk test (6MWT). This test helps us measure how well your body uses oxygen when you walk. During the test a measuring clip is attached to your finger to measure oxygen levels. Wear comfortable shoes and clothing.
Consultation. During your visit you will be seen by one of our excellent pulmonologists (lung doctors) who specializes in lung transplantation and the treatment of advanced lung disease. Although we try to honor requests for specific doctors, we cannot guarantee who will see you on the day of your visit. Other team members including a nurse coordinator, nurse practitioner, pulmonary lab practitioner and clinical assistants will also work with you during your visit.
Length of appointment. You can expect a thorough assessment of your lung condition during your visit with us, which will last 2 to 3 hours.
After your appointment. Please stop at the registration desk so we may assist you with any discharge instructions you have received and, if needed, to schedule you for another appointment.
Customer service survey. Your opinions are extremely important to us, which is why we ask you to complete the survey you receive at check-in. Your honest appraisal of your clinic experience helps us in our ongoing efforts to improve service. You may hand in your completed survey at the registration desk.
