Who receives the survey?

The Clinical & Group CAHPS survey is administered by postal mail and email. Patients are randomly selected for the formal postal mail survey through an automated process managed by Press Ganey, and all selected patients will receive a formal postal mail survey within 1 – 2 weeks of their office visit. Any patient who does not receive a postal mail survey will receive an electronic survey via email if they have provided us with their email address.

Questions Included

We use the following survey questions to calculate the star rating for each of our physicians:

  • During your most recent visit, did this provider explain things in a way that was easy to understand?
  • During your most recent visit, did this provider listen carefully to you?
  • During your most recent visit, did this provider give you easy to understand instructions about taking care of these health problems or concerns?
  • During your most recent visit, did this provider seem to know the important information about your medical history?
  • During your most recent visit, did this provider show respect for what you had to say?
  • During your most recent visit, did this provider spend enough time with you?
  • Using any number from 0 to 10, where 0 is the worst provider possible and 10 is the best provider possible, what number would you use to rate this provider?
