The Inova Juniper Program is Northern Virginia's largest provider of HIV/AIDS care. We offer all aspects of primary medical care and mental health, substance abuse, and case management services to HIV positive individuals. The Program also houses an HIV prevention program and a healthcare worker education program.
IJP is hiring for a full-time, grant-funded Program Coordinator to passionately provide case management, navigation and referral services at all Juniper locations to current and former incarcerated persons living with HIV; through our Comprehensive HIV/AIDS Resources and Linkage for Incarcerated Individuals(CHARLII) program. This role also provides backup to HIV educators/testers in the clinic and in local jails.
The Program Coordinator will partner to coordinate services, applications, referrals, and access to housing and social services. S/he will be responsible for data entry and tracking related to these services and facilitating regular evaluation of the program and reports to VDH. The Program Coordinator also serves as liaison and patient advocate on the patient care team; ensuring the meeting of patient health care needs while assisting the patient with any challenges or barriers to achieve HIV viral suppression.
The Program Coordinator must be organized and punctual, a team player, and have the skill to identify regional resources that can benefit people living with HIV and former inmates. A successful candidate will have the great attention to detail needed to manage caseloads and reporting while; also possessing the communications skills needed to liaise with staff, partner agencies, and clients.
Minimum Requirements: 2 years experience in developing, implementing and coordinating programs or projects.
