Job Description

As a Rehabilitation Registered Nurse, you will communicate effectively and work cooperatively with others while having respect for and an understanding of other clinical disciplines. To help achieve our mission, you will use an integrated approach toward achieving desired patient outcomes. Your demonstrated understanding of all Rehabilitation regulatory and payer requirements, and your ability to apply them in clinical practice on a daily basis, is of vital importance. Ensuring improvements in practice settings by assuming responsibility for self-development in lifelong learning while providing direction/guidance to others regarding practice and serving as a resource, preceptor and mentor is required. Demonstrating leadership skills in decision making and problem solving while providing knowledgeable/caring clinical practice and care coordination through an understanding of patients, families, nurses and healthcare delivery teams is essential.

Job Responsibilities

  • Demonstrates effective communication skills and assists in the resolution of conflict among healthcare team members, patients and families. Collaborates with the multidisciplinary team and incorporates their expertise to achieve desired patient outcomes. Remains informed of unit initiatives. Incorporates the outcomes of the team or committee work into practice. Delegates patient care activities and coordinates unit activities. Works with the team to achieve maximum productivity. Contacts families, physicians or other healthcare team members in anticipation of special needs of patients or families in a timely manner to facilitate effective treatment plans. Provides educational materials and methods appropriate to patients' levels of understanding and motivation.
  • Demonstrates collaboration in communication with team members and is open to feedback and receptive to change. Includes families/caregivers in education and training as appropriate. Actively participates in weekly team rounds and patient/family meetings. Demonstrates an understanding of all Rehabilitation regulatory and payer requirements and applies them in clinical practice on a daily basis. Achieves credentialing in FIM within two weeks from date of hire and annually thereafter at a passing rate greater than or equal to 80 percent. Understands how regulatory requirements affect clinical practice (e.g. three hour rule, bowel and bladder scoring, admission/discharge FIM scores). Completes accurate documentation in FIM for accurate scoring of any of the FIM items observed, for appropriate justification of score and within established timeframes. Maintains NIH stroke certification annually. Ensures improvements in practice settings by assuming responsibility for self-development in lifelong learning. Provides direction and guidance to others regarding practice while serving as a resource, preceptor and mentor.
  • Demonstrates role model and leadership behaviors consistently. Acts as a clinical resource and serves as a mentor to healthcare team members. Identifies own learning needs and develops professional goals. Seeks opportunities to obtain education to enhance practice. Collaborates in the development and implementation of educational plans for identified needs. Reviews self-development plans and professional goals. Seeks new knowledge toward the progression of professional goals. Demonstrates evidence of self-development by obtaining a minimum of six formal contact hours.
  • Maintains own Team Member Continuing Education Record. Performs comprehensive assessments by interpreting multiple, sometimes conflicting, data and synthesizing their interrelationships. Formulates individualized plans of care based on assessment findings. Interprets trends in patient populations to achieve best practice. Supports and empowers patients and families as they progress through the healthcare continuum from health promotion to end of life decisions. Advocates for patients' rights from the patient/family perspective. Counsels and acts as a resource to resolve issues.
  • Anticipates ethical, spiritual and cultural needs while intervening to maximize desired patient outcomes. Coordinates healthcare and alternate services for patient/family transition planning. Makes clinical decisions based on experience, patient population data and intuition. Promotes clinical decision making development in all team members. Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation.
  • Evaluates the outcomes of documentation and contributes to related performance improvement initiatives. Provides validation of clinical decisions for other staff members. Seeks validation from others for complex patients or unfamiliar situations. Collaborates with healthcare teams and leads other disciplines to meet desired outcomes.
  • Mentors staff in understanding and advocating for the importance of utilizing relationship based care initiatives in nursing practice. Evaluates processes to achieve desired outcomes. Anticipates patient variances and makes revisions to plans of care.
  • Participates in quality improvement initiatives. Incorporates research findings into nursing practice. Utilizes standards, guidelines and pathways for care delivery. Documents effectively/appropriately within regulatory guidelines.
