Job Description

The WOCN Coordinator communicates effectively and works cooperatively with others. Has respect for and understanding of other clinical disciplines. Uses an integrated approach toward patient outcomes. Develops and monitors standards, guidelines, and pathways for care delivery. Incorporates data and information to improve care and practice continuously, and to enhance outcomes. Ensures improvements in practice settings by assuming responsibility for self-development in life-long learning. Provides direction and guidance to others regarding practice. Serves as a resource, preceptor, and mentor. Demonstrates leadership skills in decision making and problem solving. Provides safe, therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care. Fosters high standards of practice relating to the care, teaching and rehabilitation of persons having ostomies, fistulas, wounds and incontinence. Serves as a subject-matter- expert to peers, physicians and leaders on complex clinical issues in wound, ostomy, and/or continence care.

Job Responsibilities
 
  • Consistently communicates and collaborates with the multi-disciplinary team, patients, and families. Incorporates the expertise of the multi-disciplinary team to maximize patient outcomes.
  • Delegates and coordinates patient care activities to optimize the delivery of treatment modalities.
  • Assists staff to assess the patient's educational needs.
  • Accesses resources from all disciplines to achieve optimal patient outcomes.
  • Keeps informed of operating unit/ and system initiatives. Incorporates the outcomes of the hospital, system teams, and committees into practice.
  • Serves as a change agent in the implementation of practice initiatives.
  • Participates in an operating unit team or committee that impacts practice. Integrates and links the operating unit committee initiatives.
  • Refers patients to home care, outpatient services, and support groups as appropriate.
  • Maintains effective communication across the health care continuum.
  • Develops and monitors standards, guidelines, and pathways for care delivery.

  • Leads quality improvement initiatives related to practice.
  • Analyzes practice variances, and draws basic conclusions from aggregate data.
  • Accesses resources to assist in variance analysis and implements best practices into treatment modalities.
  • Develops and monitors standards of care and assists in the formation, evaluation, and revision of procedures, products, policies, and pathways related to clinical practice, incorporating a multi-disciplinary approach.
  • Recognizes trends in patient populations, and utilizes best practice and research findings throughout this process.
  • Develops and revises patient and staff education programs. Evaluates the effectiveness of learning. Ensures improvements in practice settings by assuming responsibility for self-development in life-long learning.
  • Provides direction and guidance to others regarding practice. Serves as a resource, preceptor, and mentor. Demonstrates leadership skills in decision making and problem solving.Acts as a clinical resource and serves as a mentor to health care team members; displays leadership behaviors in practice.
  • Collaborates in the development of educational plans for staff development based on outcomes analysis. Provides formal and informal staff education opportunities.
  • Reviews self-development plan and professional goals; seeks new knowledge toward the progression of professional goals. Demonstrates evidence of self-development by participating in a minimum of 80 hours of self-development activities with a minimum of 18 formal contact hours. Maintains own Employee Continuing Education Record. Provides safe, therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care.
  • Fosters high standards of practice relating to the care, teaching and rehabilitation of persons having ostomies, fistulas, wounds and incontinence.
  • Performs a comprehensive assessment by interpreting multiple, sometimes conflicting data and synthesizing the interrelationships of the data.
  • Assesses ostomy patients and formulates a plan of care, both pre- and post-operatively, to facilitate rehabilitation. This includes stoma site marking, appliance fitting and supplier information.
  • Assesses patients with complex wounds, fistulas and percutaneous tubes.
  • Devises and implements care plans and pouching system which provides for skin protection, healing, and patient comfort.
  • Assesses incontinent patients in terms of causative and contributing factors.
  • Assists patients/staff with education in treatment techniques or selection of appropriate management, including skin care.
  • Provides nursing leadership/direction in implementation of plans for urinary and fecal incontinence.
  • Provides consultation on individual patients at risk for skin breakdown.
  • Assesses patients for causative factors and implements planned treatment modalities with the healthcare team.
  • In collaboration with the physician, provides prescribed therapy to include debridement and topical treatment modalities.
  • Administers and approves selection of cost-effective therapeutic support surfaces.
  • Leads skin prevalence study within operating unit.
  • Implements programs for maintenance of skin integrity and prevention and management of skin breakdown and monitors for compliance.
  • Anticipates ethical/spiritual/cultural needs and intervenes to maximize patient outcomes.
  • Advocates for patient rights from patient and family perspectives.
  • Provides counsel and acts as a resource to resolve issues. Evaluates and documents patient outcomes.
  • Makes revisions in the plan of care, ensuring plan effectiveness and cost efficiency.
  • Serves as a subject-matter- expert to peers, physicians and leaders on complex clinical issues in wound, ostomy, and/or continence care.
  • Performs in-services for peers, physicians and leaders.
  • Serves as preceptor for all new clinical staff coming in to the clinical field.
  • Develops and provides educational material for staff, patients and physicians.
  • Maintains abreast of current best practices in wound, ostomy and continence care as relates to their role and shares knowledge with treatment team and leadership. Applies current evidence based practices in wound, ostomy and continence care as it relates to the role.

Additional Requirements

 

3 years RN experience providing wound/ostomy care and other wound certification or ,5 years RN experience providing wound/ostomy care., Minimum 2 years RN experience in the clinical setting., Three years RN experience providing wound/ostomy care and CWS, WCC certification OR Five years RN experience providing wound/ostomy care, Three years RN experience providing wound/ostomy care and CWS, WCC certification or 5 years RN experience providing wound/ostomy care, WOCN certification

BLS required, WOCN Certification preferred
