Job Description

As a Manager of Case Management, you will actively participate in clinical performance improvement activities. Evaluating the process of initiating plans for discharge and/or transitions of care and ongoing case management needs for patients and/or families is required. Supervising staff on the coordination and facilitation of patient care progression throughout the continuum of care is expected. The Case Management Manager will also be responsible for relaying and educating on the metrics of the department such as discharges before noon, assuring assessments are completed in a timely manner and ensuring that the frontline case managers are proactively planning and executing a discharge disposition to assure a proper length of stay for patient and hospital’s wellbeing. Additionally, the Manager is also responsible for mentoring new and existing Case Managers on difficult discharges and family issues as well as working closely with the Physician Advisors on multidisciplinary rounds.

Requirements

Education:

  • Minimum for Nursing CM Mgr: BSN
  • Preferred for Nursing CM Mgr : MHA

Experience:

  • Three – five years of experience in case management or relevant nursing experience to include utilization review, discharge planning, outcomes management, transitional planning, assessment, care planning and service implementation.
  • A minimum of two years of direct supervisor/manager experience.
  • Knowledge of regulatory requirements

License:

  • RN License in VA 

Certification:

  • BLS through the American Heart Association (upon start)
  • ACM or CCM or MCG/UR

Job Responsibilities

  • Participates in the interviewing/hiring process by reviewing job profiles, reviewing candidate applications/resumes, developing behavioral interview questions, conducting and evaluating interviews in a timely fashion and providing feedback/recommendations on hiring decisions.
  • Ensures referrals are initiated to appropriate next level of care (e.g. acute care, home health care, hospice, SNF, LTAC, clinics, TCM), in a timely manner, and coordinates ordering of medical equipment/supplies for safe discharge planning or ongoing care planning.
  • Works with staff to facilitate family meetings with all patients as requested by patient, family or healthcare teams. Leads collaboration with physician and members of the interdisciplinary teams to facilitate care for designated case loads.
  • Supports the contact of families, physicians or other healthcare team members in anticipation of specials needs of the patients or families, in a timely manner, to facilitate effective treatment plans.
  • Ensures that patients/families are appropriately assessed for needs with a developed and individualized continuing care plan in collaboration with physician.
