A diagnosis of advanced heart disease, also known as congestive heart failure or CHF, indicates heart disease that has progressed to a point where traditional therapies and symptom management are no longer working.

Most often patients seen by the specialists in the Advanced Heart Failure Program are referred by their cardiologist or primary care physician.
