ElderLink is a nonprofit partnership between Inova Health System and the Fairfax Area Agency on Aging which aims to promote the well-being, safety, and independence of older adults and caregivers.

Navigating the world of services for older adults can be overwhelming. ElderLink is here to help. We listen to each unique situation and connect older adults and their caregivers to services that may help them meet their individual goals and needs.

How ElderLink Can Help You:

  • Case Management: Comprehensive care coordination to include: in-home assessment, care, planning, monitoring, family counseling, referrals, and assistance acquiring services. Affordable fee for service.
  • Bathing Services: For eligible participants who have a personal care need. Sliding Fee scale based on income. Functional and income eligibility requirements apply.
  • A Matter of Balance: A FREE evidence-based program designed to reduce the fear of falling and increase activity levels among older adults. It includes 8 two-hour small group sessions led by trained facilitators. Home safety assessments are offered

  • Chronic Disease Self-Management: A FREE 6-week workshop for adults living with chronic health conditions. It is designed to promote action planning for healthy living, well-being and self-management of one’s chronic disease. 

    Adults with ANY chronic disease, caregivers, friends, and/or relatives of a person living with chronic illness are all welcome.

  • A Walk with Ease: This FREE six-week program, created by the Arthritis Foundation, allows you to walk at your own pace three times a week. The group meets weekly to discuss successes, share challenges, encourage one another and learn information about staying healthy. Receive a free guidebook and access to online resources. Class offered via Zoom. 

    Adults with ANY chronic disease, caregivers, friends, and/or relatives of a person living with chronic illness are all welcome.

  • Bingo + Gentle Exercises = Bingocize®: Bingocize is a free evidence-based program that combines the game of bingo with fall prevention exercises (seated or standing). Come play bingo, win prizes and meet new people while learning techniques to reduce falls and increase activity. Classes meet twice a week for 10 weeks. ​​​​​​​​​​​​​​​​​​​​​

  • Geriatric Care Consults: When in doubt, call us! We are happy to listen to your situation and point you in the right direction – whether that leads you to an ElderLink program or elsewhere. This service is FREE. You can reach the ElderLink intake line at 703-324-5374. Leave us a message and a skilled Geriatric Care Manager will call you back within 24 business hours.

Inova Events

 

  • Caregiver Consultations: Support and guidance for caregivers of older adults. Up to one hour FREE consultation in office or via phone. No income requirements.

  • ElderLink Caregiver Support Call: These FREE caregiver support calls are designed to provide a convenient, safe space for caregivers to learn, discuss, and connect with one another. The call takes place on the 2nd Tuesday of each month. 

    Register for a caregiver support call 

  • Caring For You, Caring For Me: A FREE 5-week program offering support and education to caregivers of older adults. 
    This program is licensed by the Rosalynn Carter Institute for Caregiving and focuses on advocacy and care for both the caregiver and care recipient. Participants meet for 2 hours weekly and focus on a different module each session. Participants report increased feelings of confidence and competence in their caregiving role after attending the program.

ElderLink is a nonprofit program that relies on community support. Tax-deductible contributions from individuals, service groups, and local businesses help us participate in the partnership for the sake of our older neighbors. Donations may be mailed directly to ElderLink.
