Care Connection for Children is part of the Virginia Department of Health statewide network of excellence serving children and youth with disabilities. Our caring team works closely with families to coordinate care and find the best healthcare and community resources in Northern Virginia. We are committed to helping children reach their optimal health and quality of life in a caring, innovative and culturally sensitive manner.

Family-Centered Services:

Our professional case managers partner with families to:

  • Connect with healthcare resources: doctors, medicine and equipment
  • Apply for community programs and resources
  • Communicate with schools, medical providers and community programs
  • Learn about special education and how to handle school challenges 
  • Achieve successful transitions: school, medical and social
  • Provide compassionate guidance and support 
  • Bilingual staff – Interpreters available for all languages
  • Services provided regardless of income, insurance or immigration status
  • All services are FREE

What is Care Coordination?

Families raising children with special needs often have to juggle and connect all of the people and organizations involved in their child’s life.  This can cause stress.  Our team at Care Connection for Children can help relieve some of this stress by helping families to coordinate and plan care.
